Paras Health Srinagar performs life-saving surgery for critical abdominal case

Srinagar, June 09: In a landmark achievement for healthcare in Jammu and Kashmir, Paras Health Srinagar has successfully performed the region’s first combined Endovascular Stenting, Mechanical Thrombectomy, and Surgical Management of a complex case of Acute Mesenteric Ischemia (AMI).

The multidisciplinary effort was led by Dr. Irshad Ahmad Banday (Interventional Radiologist) and Dr. Ishaq Nabi (General Surgeon), with critical support from the Departments of Diagnostic and Inter-ventional Radiology and Surgery.

A statement issued here said that the patient, a 45-year-old man with a history of hypertension and no known prior vascular disease or diabetes, was initially misdiagnosed as a case of simple gastritis at a government institute and sent home. He arrived at Paras Health Srinagar when the symptoms in-creased, including persistent pain, vomiting, and bleeding per rectum. Despite initial hesitancy from the patient and family, persistent counselling and vigilance by Dr. Ishaq Nabi ensured a timely CECT abdomen, revealing a rare and dangerous Superior Mesenteric Artery (SMA) dissection with com-plete distal thrombotic occlusion and non-enhancing bowel loops.

Recognisng the urgency, the med-ical team acted swiftly within a critical 4-hour window from diagnosis to intervention. An emergency endovascular procedure was performed in the DSA lab. Dr. Irshad Ahmad Banday and his team placed a balloon-mounted stent to treat the proximal SMA dissection and executed manual mechan-ical thrombectomy using the Penumbra device to clear the distal thrombus. A follow-up angiogram confirmed full restoration of blood flow to the bowel.

Subsequently, a diagnostic laparoscopy performed by Dr. Ishaq Nabi and Dr. Irshad Ahmad Banday (Surgeon) confirmed bowel viability, revealing edema without signs of transmural ischemia. The patient was then managed in the Surgical ICU (SICU) and discharged on the 7th day in stable condition, fully relieved of symptoms. A repeat CECT scan 24 hours later showed patent stent placement with normal enhancement of the vasa recta and bowel segments. “This case represents a clinical milestone not only for Paras Health Srinagar but for the entire healthcare landscape of Jammu and Kashmir. The successful endovascular and surgical management of acute mesenteric ischemia is a testament to our hospital’s growing expertise in advanced emergency care. Timely diagnosis and treatment is critical in these cases,” said Dr. Irshad Ahmad Banday, Interventional Radiologist. Dr. Ishaq Nabi, the lead surgeon, added, “Acute mesenteric ischemia has a notoriously high mortality rate if not addressed rapidly. It was a race against time, and only through seamless teamwork and persistence were we able to achieve this outcome. This case reaffirms the value of clinical vigilance and patient-centred care.” The success of this high-risk case was made possible by the collaboration between Emergency Medicine, Anesthesia (Dr. Nasir), Critical Care (Dr. Imran), Cardiology (Dr. Fayaz and Dr. Suhail Maqbool), Radiology, and Dietary Services. It marks a new chapter in minimally inva-sive and hybrid interventions for critical abdominal emergencies in the region.
